Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
We are considered a "contracted provider" for many common dental insurance companies such as
Delta Dental, Met Life, Cigna, Aetna, Principal, United Concordia, and First Dental Health. This means we charge you the discounted fee's that we are contracted to charge.
For your convenience we accept Visa, MasterCard and Care Credit. We deliver the finest care possible at the most reasonable cost to our patients, therefore payment is due at the time services are rendered unless other financial agreements have been made in advance. If you have questions regarding your account, please contact our office at 209. 839. 8594. Many times, a simple telephone call is all that is needed to clear any misunderstandings.
Please remember you are fully responsible for all fees charged by this office regardless of your dental insurance coverage.
We will send you a monthly account statement. Most insurance companies will respond within 4 weeks. Please call your insurance company if it has been more than 4 weeks. Any remaining balance left after the dental insurance pays is your responsibility. IF needed, we can establish a payment plan with Care Credit for you. IF you would like to apply yourself, you can go to www.carecredit.com and apply. They will have an answer within seconds. Simply give our office a call and we can bill them for you.
We bill all dental insurance companies as long as they allow you to choose your own dentist. There are some insurance companies that assign you to a dental office, so make sure to call your dental insurance company and make sure you can choose.
General Dental Insurance information: Your dental plan is a form of compensation provided by an employer. You can expect the carrier to reimburse you for a portion of our fee. That portion is determined by the contract between the employer and the insurance company. The higher the premium paid by the employer the more generous the reimbursement by the dental insurance company.
Although we have no influence on the contractual agreement between the employer and the dental insurance company, we do strive to help you receive the maximum benefit possible. We send narratives, reports, dental x-rays, intra- oral photos, and anything else we have to help with the payment of the claim on your behalf.
